NOTE: We publish this information free of charge on our website, so family and friends may light a candle, send flowers or offer condolences.

  • Remains between 250 - 350 lbs is an additional $200
  • Remains between 350 - 450 lbs is an additional $350
  • Remains between 450 lbs and above is an additional $500
  • Cremains Shipping and Handling will be an additional charge of $150 (basic plastic urn)
guidelines to follow
  1. Notify the Hospital, Hospice, or Nursing Facility to call Funeral Home at 513-923-1110 once your loved one has passed. (This information can be provided to Facility Institution before or after.) We do not need any paperwork in order to pick up your loved one, just verbal authorization from the Next of Kin or Person in Charge.
  2. Before or after your loved one passes, please print the forms below, fill them out by hand and fax, email, or deliver by postal service to 1712 Queen City Avenue, Cincinnati, OH 45214 (You may also take a picture of the forms with your smartphone & send via text)
  3. If you do not have access to these forms, you may make an appointment with one of our Licensed Funeral Directors to finalize arrangements.
  4. Payment may be made at the Funeral Home in the form of cash, cashier's check, money order, or any major credit card.

Basic Services of Funeral Director and Staff, removal from place of death into our care (within a 25-mile radius), transportation to crematory, cremation container, cremation process, all necessary authorizations, and cremation fees; 30-minute visitation followed by one-hour memorial service (M-F) in the church chapel before 4 p.m., funeral home equipment & staff, guest book; one (1ct) certified death certificate; fifty (50ct) memorial cards or memorial programs, standard urn, includes weight up to 250 lbs.

Additional charges: $300 upcharge for Saturday, Sunday, and Monday through Friday after 4 p.m. (evening) memorial services.

Options: Courtesy view, urn and keepsake jewelry, memorial tribute blanket, DVD tribute, urn floral arrangement, photo keepsake.

Additional charges: $300 for Saturday, Sunday, and Monday through Friday after 4 p.m. (evening) services, additional visitation hour (s) additional death certificate(s), upgraded casket spray, memorial tribute blanket, DVD tribute, upgraded funeral programs or memorial cards, urn and keepsake jewelry, dove release, etc.
